In view of the fact that in this office we also perform some purely elective surgery, the fees for this type of surgery are paid for in advance of the surgery, unless covered by your insurance carrier. If surgery is indicated, the patient is responsible for furnishing insurance claims to the office prior to surgery.

We would appreciate it if you would be prepared to take care of your account at the time of the office visit. This also includes any insurance co-payments. Unless special arrangements have been made with the office administrator, payment is due at time of office visit.
